Community Medical Center is pleased to announce the Transfer Center is coming back in-house! We have heard your feedback, and we welcome the opportunity to partner with you in caring for your patients. Our in-house transfer staff is available 24 hours a day, seven days a week and is committed to serving providers and patients.

  • Transferring provider’s name, facility and contact information for call-back or disconnect purposes
  • Patient’s name, age, date of birth
  • Clinical status
  • Fax face sheet to 406.327.4232

How it Works

  1. Our House Supervisor will connect with the appropriate provider to accept the patient.
  2. Our House Supervisor will notify you with bed assignment if applicable and contact information for giving report.

For consultations we encourage you to utilize the on-call schedule to connect directly with our on-call specialists.
